Welcome to the Personal Planning Guide designed by ACHIEVA Family Trust. We hope that you and your family find this a useful planning vehicle. We welcome your feedback.

The Personal Planning Guide serves as a repository of information about an individual that can be passed on to future caregivers. Think of the Guide as a snap-shot of your family member, containing all the information that makes him/her one of a kind. The more detailed the Guide, the more defined the picture is that you will be leaving for other caregivers to understand your family member. The Guide can be updated and changed as your family member's needs and lifestyle change. We recommend keeping the website with your username and password in a safe place, as well as, giving it to a family member or a potential future caregiver for emergencies. In order to assure that the Guide will survive you, it is also recommended to leave the website with your username and password to the executor of your estate.

The Guide provides several different categories to assist in all stages of life. Some examples of the categories are: General Information, Family & Friends, Likes and Dislikes, Life & Learning, Health, Finance, Final Arrangements. These categories are also listed in the tool bar, which will be located at the left of your screen once you log-in.

The Personal Planning Guide is to be used during the lifetime of your family member; when the family member passes away, please notify us by using the site comments section so that he/she can be removed from the guide.
